If you’re an admin, you can create and manage review cycles for your company. Our handy templates give you a head start on the settings and questions!

You can use Breezy’s review templates as-is, or customize any of the questions and settings.


Basic Review template

By default, the Basic Review template includes:

  1. 4 self-evaluation questions for employees:

    1. Describe the work or contributions you are most proud of in this review period.

    2. What are 2-3 ways you want to grow professionally or personally?

    3. Which of our company values did you live best in the last few months?

    4. What project/goal(s) would you like to focus on in the next quarter/six months/year?

  2. 4 employee-evaluation questions for managers:

    1. Describe a meaningful contribution this employee has made since the last review.

    2. Which company value(s) have you seen this employee demonstrating well in the last few months?

    3. What are some areas of focus/values that could benefit this employee?

    4. What are some positive interactions you’ve noticed between this employee and others?

  3. 4-point scales for both employees and managers to rate the employee’s performance against active employee and department goals:

    1. Opportunities to Improve

    2. Consistently Meets Expectations

    3. Often Exceeds Expectations

    4. Almost Always Exceeds Expectations


360 Review template

By default, the 360 Review template includes 4-point scales for employees, peers, and managers to evaluate each other on:

  1. Company culture

    1. Requests/accepts feedback and evaluates past decisions to improve future outcomes.

    2. Fosters an optimistic, respectful, collaborative work environment that inspires others to contribute their best.

    3. Embodies our organization’s core values.

  2. Delivery

    1. Communicates effectively with the immediate team, the wider team, clients, and in all mediums.

    2. Manages emotions and stress effectively while fulfilling commitments in the face of unexpected challenges.

    3. Generates creative solutions while making effective use of time.

    4. Contributes to and evangelizes the company’s long-term goals and vision.

  3. Leadership

    1. Takes initiative, requests help, leads, guides, and delegates appropriately while taking responsibility for their own actions.

    2. Actively seeks opportunities to improve themselves and internal processes.

    3. Proactively identifies and addresses problems before they escalate.

    4. Considers all sides of an issue and the short and longer-term impact of decisions.


Skills Assessment template

By default, the Skills Assessment template includes 4-point scales for both employees and managers to evaluate the employee’s:

  1. Communication skills

    1. Articulates thoughts and ideas clearly and concisely in written or verbal form

    2. Actively listens, understands, and interprets non-verbal cues well

    3. Adapts communication style to suit different audiences

  2. Teamwork

    1. Shares ideas and resources

    2. Takes on different roles and responsibilities

    3. Supports team members as needed

    4. Builds trust and rapport with colleagues

    5. Communicates effectively with the team

    6. Is receptive to feedback

  3. Problem solving and creativity

    1. Logically and systematically identifies root causes to problems

    2. Uses creativity to generate and implement effective solutions

    3. Demonstrates critical thinking, research, data analysis, and innovation

  4. Time management skills

    1. Proactively plans, prioritizes, and organizes

    2. Stays focused and avoids distractions

    3. Seeks assistance or delegates tasks when appropriate

  5. Leadership skills

    1. Provides direction and guidance

    2. Manages resources and people effectively

    3. Makes difficult decisions when necessary

    4. Fosters a positive and inclusive team culture

  6. Adaptability

    1. Works effectively in diverse environments

    2. Flexes to changing priorities

    3. Learns new skills and systems quickly
